SmartVest is available by prescription only. Request a Patient Packet, with forms included, to bring to your next appointment to discuss symptoms and if you could have a chronic lung condition like bronchiectasis. To properly diagnose bronchiectasis, ensure your doctor requests a HRCT scan of your lungs and airways.
Our support team is ready to walk you through the process. Whether you want to discuss your symptoms with one of our Respiratory Therapists, want to learn more about the prescription process, or questions about insurance, we’re here every step of the way.
We require minimal information to begin the prescription process. Ask your doctor to complete the required forms and fax all completed documents to Electromed at 866.758.5077 or easily ePrescribe via Parachute Health.
Download prescription forms to get your patients on track with SmartVest HFCWO therapy. Each form is designed with input from clinicians and payers to capture prescription details efficiently and accurately.
Our reimbursement team is available to assist your healthcare team throughout the documentation and reimbursement process to ensure your patients’ SmartVest is delivered safely to their door, and they’re able to start their HFCWO therapy immediately with the help from a personal Registered Respiratory Therapist (RRT).
